At Centennial Place, our staff is dedicated to providing services that will enable you to live your life to the fullest. With aging comes increasing frailties and new challenges. We understand that. It is our role to provide a supportive, home-like environment that ensures the maximum independence, dignity and wellness of every one of our residents. We look after the details so that you can maintain your lifestyle in a safe and comfortable setting.

Individual Choices
Our commitment is to treat every one of our residents as an individual and with this in mind we tailor programs to meet specific needs. We believe that each resident has the right to make decisions about their own care and we encourage individual control over the choices encountered in daily life. Our emphasis is on the self-direction and the dignity of each resident, regardless of their physical condition.

Holistic Care
At Centennial Place, our team of professionals provides a wide range of services to care for the whole person - mind, body and spirit. "Wellness" is more than just attending to physical care needs, it means fulfilling emotional and spiritual needs as well. At Centennial Place, we emphasize what our residents can do—and what they choose to do—in striving to achieve the maximum level of independence and wellness.

Family Matters
Understanding the importance of family involvement is part of what makes Centennial Place a special place to live. We welcome family participation in every aspect of our residents' lives, from dining to activities to resident care. An open door policy invites family and friends to become a part of our community.

We recognize, too, that a resident's family is also an integral part of our care commitment. Through family support groups, newsletters and a variety of information brochures and conferences we keep families informed. We encourage ongoing dialogue to ensure that all needs are being met. At Centennial Place residents and their families are an important part of our "care team".

Nursing Support
We have experienced nursing staff on duty 24 hours per day to ensure our residents' health and well-being. Under the direction of the Director of Care, our care team includes Registered Nurses, Registered Practical Nurses and Personal Support Workers who follow the professional standards of practice of the College of Nurses of Ontario. We administer medications and treatments as ordered by physicians, coordinate therapy services, and provide assistance with the activities of daily living, including bathing, dressing, and hygiene.

Our staff is always right around the corner. Each room has an emergency "call system" to enhance safety and security. In addition, at Centennial Place our care teams are assigned to specific "houses" so that they become familiar with our residents, including their care needs and the rhythms of their daily lives. Our staff will also offer counseling and support to residents and their families regarding changing health conditions. We will work closely with you and your family to determine the amount of care you require.

Medical Care
For added convenience, residents may use one of our advisory physicians who visit Centennial Place each week. Routine lab tests, blood tests and some x-ray assessments are arranged on-premises for your convenience. All prescription drugs are coordinated through our nursing staff and are delivered daily. In addition, a local dentist and denturist can provide basic assessments on-site (using standard fee schedules).

Dementia Care
Centennial Place provides a wide range of care services, including complex health care and dementia care. "Heritage House", one of our four home-areas, is dedicated to providing a comfortable and safe environment for those who require specialized dementia care services. Our highly skilled staff ensures that residents with memory impairments live in an atmosphere that is reassuring and supportive of their needs and of their identity. Our innovative programs stimulate, nurture and preserve their quality of life. In Heritage House we help to maintain a sense of normalcy and routine in our residents' lives and we respect them for the individuals they are.

Wellness Centre Services
We organize a variety of on-site therapy services to enhance the wellness of our residents and to preserve their independence. Individual or group programs are arranged depending upon needs. Therapy programs such as physiotherapy, occupational therapy and speech therapy are provided by our staff or accessed through Home Care. Some additional "spa" services are also available on-site, including hair salon (or barber), foot care, massage therapy and health education. Standard professional fees apply.

Meals and Dining
We believe that dining should be a pleasant experience for every resident - every single day. Meals are an important part our residents' lives. They encompass both a social and a nutritional element that is crucial to well-being. At Centennial Place, we pride ourselves in serving delicious home-style meals, prepared in our own kitchen, in the tradition of a country home. To ensure each resident's comfort and to promote social interaction, each of our "houses" has a pleasant dining area specifically for its own residents and their guests. We also offer a private dining area for family get-togethers and special occasions.

Our menus are developed with our residents' nutritional needs and their tastes in mind. Full meal service is provided three times each day, including a choice of entrées, vegetables, beverages and desserts. Alternate selections are also available. In addition, nutritious snacks and beverages are provided between meals and through the evening. With input from our residents, a registered Dietitian ensures that our rotating menus meet nutritional needs and offer plenty of variety - including many traditional favourites. Special therapeutic diets, such as diabetic and texture-modified diets, are provided as required. If a resident is ill or not up to dining with others, a meal will be provided in his or her room.

Housekeeping Services
Our attentive staff carries out regular housekeeping and maintenance duties to make sure that each resident's environment is clean, safe, and comfortable. Each room receives a light cleaning daily. A more thorough cleaning is done every week, or more frequently if required. A complete laundry service is provided by our staff right on the premises. Dry cleaning and mending services can also be arranged.

Activities

At Centennial Place, our goal is to enable you to maintain an active, enjoyable lifestyle. We offer a full range of group and individual programs for your recreational, intellectual, and cultural enjoyment.

To encourage participation, our staff prepares a varied calendar of events full of stimulating activities. Crafts, cards, bingo, gardening, exercises, pet therapy, movie nights, word games, live entertainment, shopping trips, men's club, sensory stimulation, music and art therapy, and special theme dinners are just some of the activities available. We encourage active involvement in all programs, but residents may choose to participate or not as they see fit.

In addition, residents are encouraged to remain involved with their family, their friends, and their community. Visits from members of local organizations, churches, school groups and choirs are popular events and foster a sense of connection with the community. Holiday parties and the opportunity for excursions to local events contribute to community spirit. Transportation and staff support is available for group outings. Family participation in all the events at Centennial Place is welcomed and encouraged.

Spiritual Programs
Centennial Place has a lovely on-site chapel and offers a supportive program for those who wish to continue an active spiritual life. Residents may choose to go to a church service in their community or attend our weekly on-site chapel service, provided by local church leaders. In addition, our Spiritual Care Coordinator is available to assist in spiritual care discussions with you or your family. Celebration of religious holidays from all faiths is encouraged.

Short Stay/Respite Accommodation
Our full range of programs and services is available to residents on short stay placement. For caregivers, we are here to offer you relief from your care-giving responsibilities while tailoring our services to the specific needs of your loved one. Making Centennial Place home for a short time also gives a resident first hand experience of our home-like environment and our skilled staff who are dedicated to maximizing independence, dignity, and wellness.
